Choose the pathway that matches what you need. Each form sends the details to the Instrumental Music Department for review and follow-up.
Immediate safety emergency?
Tell a staff member right away. For all other lost items, damage, instrument problems, and support needs, submit an issue ticket instead of interrupting instruction, rehearsal, lunch, or preparation time.

Request Music Department Space
Request the Band Room, Back Room, or Storage Room.
- Submission does not grant access
- Written approval is required
- Students need approved adult supervision
Request a Performance
Submit an event for professional review by the Director of Instrumental Music.
- Generally submit at least six weeks ahead
- Review the program calendar first
- Do not advertise participation before written approval
Performance requests are requests for review only
Submitting the form is not a commitment, calendar hold, tentative booking, or approval. The Director will review the program calendar, student availability and readiness, staffing, transportation, supervision, equipment, event logistics, costs, and required approvals before responding. Do not advertise the ensemble’s participation until written approval has been received.
Policies & Expectations
Submitting complete information helps the department respond efficiently. Written approval controls access and participation.
Property & support
- Return department property with all issued accessories.
- Report damage, loss, or missing parts promptly.
- Identify ownership so authorized repair and replacement procedures can be followed.
- Financial responsibility follows applicable NHS and NLMUSD policies; this system does not invent fees.
Music department spaces
- Only the approved space and equipment may be used.
- Food and drinks are prohibited except bottled water unless specifically approved.
- Restore the room, secure doors, and report damage or missing property immediately.
- Instructional and program needs take priority.
Performance review
- An open calendar does not guarantee availability.
- Approval depends on students, staffing, supervision, transportation, equipment, logistics, costs, and required approvals.
- Donations and reimbursements follow school and district procedures.
External promotional events
- Hosts may be asked about transportation, meals and water, parking and loading, equipment, facilities, recognition, admission, and program support.
- A donation is not automatically required for NHS or community-service appearances.
- Business and promotional requests are evaluated professionally because program resources have real costs.
